Where this comes from, and what it cannot tell you
Every facility here is one Indiana actually licenses. The list is the state's own, not a
directory: commercial senior-living sites list the facilities that pay them for referrals,
so the places nobody is marketing to you simply do not appear. 739 facilities are
listed, generated 2026-09-09.
- The list and the inspection history come from the Indiana Department of Health's
public consumer reports — bed counts, current licence, who the administrator and director
of nursing are and when they started, substantiated complaints, and every state survey
with its full report as a PDF.
- Star ratings, staffing hours, turnover and fines come from Medicare's Care Compare
data, and cover nursing homes only. Assisted living and memory care are licensed by
the state and carry no federal rating at all. A blank rating on an assisted living
facility means no such rating exists — never that it scored badly.
- Drive times are real routing, not a circle on a map: an hour west of McCordsville, Indiana is
deep into Indianapolis, an hour east is open highway.
- Memory care is a stated signal, not a complete one. Indiana licenses memory care as
ordinary residential care, so no authoritative list exists. A facility is flagged here
when it enrolled with Medicare under a dementia speciality or says so in its name. Plenty
of others run a secured unit without either — ask.
- Independent living is not licensed by anyone, so no complete list of it can be
built. The campuses flagged here are the ones Medicare records as continuing care
retirement communities, which do include it.
None of this replaces walking in unannounced, at a mealtime, on a weekend. It is meant to
tell you which doors are worth knocking on and what to ask when you get there.
Ratings and notes
The facility list above is public information and anyone may read it. The stars and notes
are not: they are the family's own assessment, and they are kept behind a shared
passphrase. Sign in at the top of the page and everything you rate is saved on the
server and follows you to your phone; without it, anything you type stays in the browser
you typed it in.
